Health Records and Illnesses

Please complete an emergency contact form and return it to the office. In the event of an emergency the school must be able to contact you or another responsible person.

Please notify the school of any conditions your child has that could result in an emergency situation with your child. (Allergic reactions to insect bites, peanuts, or medications; seizures; diabetes)

School Entry Requirements

Immunizations must be kept current according to the department of Public Health. All immunizations must be up to date for children to attend school. Verification of vaccine must appear on the health record. Please notify the school of any vaccine that your child receives.
